Fairness in Women's Sports

The debate over transgender athletes gets loudest at the exact moment it stops being about athletes. Here is what the physiology, the case law, and the newest performance research actually show, separated from the image of a grown man lining up against a ten-year-old that both sides keep reaching for.

The concern is real. So is the noise.

Male puberty produces measurable physiological changes: more muscle mass, higher bone density, greater lung capacity, and an aerobic ceiling most women's categories were built to separate out. That's not a talking point, it's the reason sex-segregated sport exists in the first place, and pretending otherwise doesn't serve anyone. A 2021 systematic review in Sports Medicine on testosterone suppression and performance found that many of those advantages persist, to varying degrees, after transition, particularly in strength-dependent sports.1

What doesn't hold up is the image built on top of that fact: an adult man, fully developed, lining up against a girl. Before puberty, children are essentially equal athletes regardless of sex. The performance gap opens somewhere between ages ten and fourteen, with enormous individual variation on both sides of it. A transgender girl on a rec-league soccer team is a child playing with other children, not a threat to competitive integrity.

The honest question is about elite, strength-dependent competition. The policies actually being written mostly land on ten-year-olds.

What the newest data shows

More muscle, not more medals

The research is more complicated than either side's slogans. Transgender women on hormone therapy do retain more lean muscle mass than cisgender women, a point the inclusion side spent years disputing and eventually had to concede. But retained muscle and retained advantage turn out to be different things. Follow-up studies after one to three years of hormone therapy have generally failed to find a significant performance gap between transgender and cisgender women in strength and endurance testing, which cuts against the assumption that more muscle mass automatically means more speed or force on the field. The caveat matters: most of this research covers short timeframes, non-elite athletes, and small samples, so it settles far less than either side would like.

The clearest test case remains the 2022 NCAA women's 200-yard freestyle, where Lia Thomas and Riley Gaines tied for fifth place, half a second behind the winner and three other cisgender women. An "unbeatable" biological advantage doesn't produce a tie for fifth. It's also a reminder that elite outcomes are decided by far more than any single variable, hormonal or otherwise.

The categories were never as clean as the rulebook implies

Women's sport has a long, uncomfortable history of testing women who were never transgender at all. Spanish hurdler María José Martínez-Patiño was stripped of her title in 1985 after a chromosome test revealed androgen insensitivity syndrome, a condition that meant her body couldn't use the testosterone the test treated as proof of an edge. She was reinstated in 1988, and the episode helped push track and field to drop mandatory sex testing. South African runner Caster Semenya and Indian sprinter Dutee Chand faced similar scrutiny decades later over naturally occurring testosterone levels, neither is transgender. Geneticist Andrew Sinclair, who co-discovered the SRY gene now used in Olympic eligibility testing, has publicly objected to that use of his own research, pointing out that a positive test only shows the gene is present, not that it ever activated or produced any advantage.2

Where the rules actually stand

From testosterone caps to a genetic switch

The International Olympic Committee allowed transgender women to compete in the female category starting in 2004, first under a surgery-and-hormone-therapy requirement, then under a testosterone ceiling after 2015. That framework held for roughly two decades without producing the wave of dominant transgender athletes the debate assumes. In March 2026, under new IOC leadership, the policy reversed course entirely: eligibility for women's Olympic events is now restricted to athletes who test negative for the SRY gene in a one-time genetic screen, with narrow exceptions for a few specific intersex conditions, taking effect at the 2028 Games.3

On the school-sports front in the United States, the Supreme Court settled the question differently than at the Olympic level. In June 2026, ruling together on cases from Idaho and West Virginia, the Court upheld state laws barring transgender girls from girls' school teams, six to three on the constitutional question, while all nine justices agreed the bans don't violate Title IX.4 The ruling didn't declare the science settled; it handed the question to individual states, meaning eligibility can now depend entirely on which side of a state line a family lives on.

Sport-specific, evidence-based eligibility rules at the elite level are defensible. A single genetic switch applied to every sport, and blanket bans applied to every ten-year-old, are not the same policy.
